Sclerotherapy is a medical procedure used to treat varicose veins and “spider veins.” A medicine is injected into the vessels, which makes them shrink. It is suitable for most children and young adults with vascular or lymphatic malformations. In adults, sclerotherapy is often used to treat varicose veins.

Varicose veins differ from spider veins in a number of ways. Varicose veins are larger — usually more than a quarter-inch in diameter, darker in color and tend to bulge. Varicose veins are also more likely to cause pain and be related to more serious vein disorders.

The precise origin of the disorder is unknown, but reports have shown that heredity, pregnancy, hormonal changes, weight gain or leg injury may influence the occurrence of leg spider veins. Spider veins appear in both men and women, but more frequently in women. Female hormones may play a role in their development. Puberty, birth control pills, pregnancy, or hormone replacement therapy may contribute to them. They may also appear after an injury or as a result of wearing tight girdles or hosiery held up with elastic bands. Varicose veins occur mainly from genetic susceptibility.

During a sclerotherapy procedure, the physician uses a thin needle to inject a solution into the unsightly veins. That solution causes the blood in the veins to lose its color. The veins cease to appear purple, red or blue.

Sclerotherapy is performed in a doctor’s place. In most cases, a 23% sodium chloride sterile solution mixed with lignocaine (a local anesthetic) and heparin is injected into a very fine needle directly into the blood vessel, using a very fine needle. A solution without lignocaine is available for people who are allergic to lignocaine. The number of varicose veins injected in one session is variable, depending on the size and location of the veins, and the patient’s overall medical condition.

A substance is injected which makes the vessels in the malformation shrink. The doctor uses an ultrasound scan to guide a needle into the malformation and then injects a substance, which shows up on an x-ray. This should show the size of the malformation and whether the blood vessels link up with important veins. The doctor will then be able to inject the substance to shrink the vessels. This causes blood clots and scar tissue to form in the malformation, which eventually leads to a gradual shrinkage.

Mild discomfort may occur, and a cramping sensation may be felt for 1 to 2 minutes when larger varicose veins are injected. The sclerotherapy procedure itself takes about 30 to 45 minutes.

The real advantage of sclerotherapy is that it causes minimal interruption to your lifestyle, is virtually painless, is extremely safe, has very few complications and can be done out of the hospital without loss of time at work or from social or sporting activities. Most treatments are spaced 2 to 3 weeks.

One can expect a good result with an 80% to 90% improvement in the cosmetic appearance of the leg.
